Required Records:

• All maintenance invoices and receipts
• Pre-trip and post-trip inspection logs
• PM service records
• Repair documentation
• Parts purchase records
• Tire purchase and replacement records

Record Retention: All maintenance records must be retained for a minimum of 12 months and made available upon request to Delta Advanced Trucking, Inc. and regulatory authorities.

Owner-Operator acknowledges and agrees to:

• Pay all costs associated with vehicle maintenance and repairs
• Maintain adequate insurance coverage for physical damage
• Keep vehicle in safe operating condition at all times
• Ensure vehicle meets all regulatory requirements
• Reimburse Company for any costs resulting from maintenance neglect

Owner-Operator commits to:

• Follow all manufacturer recommended maintenance schedules
• Use qualified mechanics and service providers
• Use OEM or equivalent quality parts
• Address safety-related issues immediately
• Maintain vehicle cleanliness and professional appearance

In case of breakdown or maintenance emergency:

• Notify Safety Department: (330) 555-0102
• Document all emergency repairs with photos and receipts
• Submit emergency repair reports within 24 hours

I, Robert Johnson, acknowledge that I have read, understand, and agree to comply with all maintenance responsibilities outlined in this document. I understand that as an independent contractor owner-operator, I am solely responsible for all maintenance, repairs, and associated costs for my equipment.

I understand that failure to maintain my equipment in safe operating condition may result in termination of my lease agreement and loss of dispatch privileges.
